Obtaining common carotid doppler profile is easy to do for most patients in the ICU, learning curve is fast. This paper shows a strong agreement between cardiac out measurement by common carotid artery doppler and invasive gold standard of pulmonary artery catheter or Pulse contour analysis.  

Comment: I plan to use this method for my patients with shock and septic shock to assess if they are fluid- responsive or not. The CO will be measured before the fluid challenge of 250 to 500 ml, then after the challenge, a 10% increase in CO, confirms that the patient is fluid responsive. 

Procedure: position the patient in a 30-degree upright. Obtain a transverse view of CCA at the thyroid level, measure the diameter, and confirm the measurement on the longitudinal view. Using pulse doppler mode, 1 mm sampling box, obtain doppler profile for at least 5 cardiac cycles, then measure time velocity interval (TVI) , 

Flow volume= TVI X D,   CO= Flow volume X 10
